Are you an experienced Health Care Professional with a passion for seniors’ health? Are you looking for an opportunity to work closely with clients/families as they navigate the healthcare system when moving towards a Designated Living Option (DLO)? Are you someone who prides themselves on their collaborative abilities in working with interdisciplinary professionals and community resources? If so, the opportunity to work as a Placement Case Manager with the North Zone Seniors Health team may be for you! Reporting to the Placement Manager, you will be working with an interdisciplinary team helping clients and their families’ transition to a DLO. You will receive client referrals for access to a DLO, confirm care level based on the homecare assessment, approve and waitlist clients and coordinate admissions to DLO. You will utilize your fact-finding skills to search out information from different resources to assist and educate the client and their families to navigate the healthcare system and plan organized transitions through the continuum of care. You will demonstrate skills in problem solving, case management, creating and maintaining interpersonal relationships, and collaboration with interdisciplinary teams and leadership. As our Placement Case Manager, you will have a passion for patient- and family-centered care, advocating for your client while working to meet their needs or their family’s needs and understand acute and continuing care processes and objectives. About Job:

As the Case Management Coordinator, you will be responsible for facilitating and coordinating client transfers. You will receive client referrals and confirm care level based on provincial guidelines and clinical judgment. In consultation with the client, their family and the care team, you will approve appropriate referrals. As the Case Management Coordinator, you will be responsible for maintaining waitlists, negotiating space offers with clients, and coordinating admissions. You will act as a liaison between the interdisciplinary team and service providers to support the client and family to navigate the healthcare system. As the Case Management Coordinator, you may be responsible for addressing delays related to system, client or family, and/or government or community programs, and you will be required to operate within provincial policy and procedures while maintaining a client-centered approach in consideration of system resources.

Required Qualifications:

Completion of a bachelor’s degree in a health related field. Active or eligible for registration and practice permit with the applicable college.

Additional Required Qualifications:

Five (5) years relevant healthcare experience with one (1) year Seniors Health experience. Home Care, Seniors Health, dementia care, discharge planning, and experience with placement or working in DLO environment.

Preferred Qualifications:

Knowledge of computer applications (Netcare, Meditech, MS Office); demonstrated competency with RAIHC; AHS McMaster Case Management; Supportive Pathways Training and knowledge of cognitive status and functioning; conflict resolution and negotiation skills; experience with clinical assessment; knowledge of applicable legislation, programs and community resources; and demonstrated ability to navigate the healthcare system and advocate for clients/families.
