Job Description


Job Summary

The Patient Care Coordinator is responsible for promoting and restoring patients’ health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.

Specific Tasks

Patient Care and Monitoring

  • Record patients’ medical information and vital signs. Take history of new patients.
  • Conduct Pulmonary Function Tests (PFT) and International Normalized Ration checks.
  • Accompany patients to clinics & hospitals for investigation and treatment.
  • Monitor, record, and report symptoms or changes in patients’ conditions.
  • Monitor all aspects of patient care, including diet and physical activity.
  • Collect results from different laboratories and clinics as well as other medical service providers – CT scan, MRI, etc…)
  • Conduct home visits and dress the wounds of post operated patients that are non-ambulatory.

Communication and Coordination

  • Communicate with patients and patient’s family to schedule appointments for doctors’ visits and to provide nursing care.
  • Prepare patients for and assist with examinations or treatments.
  • Schedule patients for appointments and tests and ensure that follow up calls are made in a timely manner.
  • Coordinate and facilitate outreach campaigns.

Documentation and Inventory Management

  • Prepare surgical files and ensure that all needed x-rays are available. Properly file x-rays, ECHO and other tests.
  • Maintain nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.

Clinical Assistance and Compliance

  • Assisting Dr. Hodes at Clinic.
  • Protect patients and employees by adhering to policies and protocols, medication administration and storage procedures, and controlled substance regulations.
  • Maintain patient confidence and protects operations by keeping information confidential.
  • Maintain a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Perform other duties as directed by the supervisor.

Salary Range: Negotiable, depending on experience and qualifications.

Requirements

Education: Bachelor’s degree in nursing or a related field.

Experience: Minimum of 2 years of nursing experience, preferably in a clinical setting.

Licenses/Certifications: Valid license to practice medicine in Ethiopia.

Skills:

  • Strong communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team.
  • Proficiency in medical software and equipment.
  • Strong organizational skills and attention to detail.
  • Ability to handle multiple tasks and prioritize effectively.
